WORKPLACE ACCIDENTS

They Cost More Than You Might Think

The indirect costs of industrial accidents which take into account the sometimes immeasurable costs of lost production and efficiency on a company-wide basis-can be several times the costs of calculable workers' compensation and employee disability payments. These costs, however, are unnecessary. A recently released study notes that about 85 percent of all workplace injuries and fatalities can be avoided by making sure employees have been properly trained for their jobs-by introducing and requiring all employees to adhere to safe workplace practices, and by getting managers as well as the operational staff to make a genuine commitment to worker safety.

The accident took place in an instant. The worker received prompt first-aid attention and the injury was treated. The after-effects of the accident, however, transpired more slowly. Workers who witnessed the scene were too shaken up to resume their duties. Production stopped until the damaged equipment was cleared and replaced with new machinery. Managers reassigned the injured worker, called in a replacement, and explained the duties. The supervisor filled out a lengthy accident report and several regulatory forms. The plant owner prepared for possible litigation. What are the cost implications of these interrelated events? In most cases, such accidents and the accompanying loss of production and efficiency can be avoided by establishing manager/employee safety committees, instituting regular training programs and inspections, conducting emergency drills, and other steps. Certainly the most quantifiable benefit resulting from the successful introduction of a safety program is a reduction in workers' compensation insurance rates. Less measurable benefits, however, involve the avoidance of the indirect costs of an accident

Indirect costs

The indirect costs of an accident take into effect the sometimes immeasurable costs of lost production and efficiency on a company-wide basis. They include the following costs Wages for lost time of uninjured co-workers. Workers adjacent to the accident scene who stop their work to watch or offer assistance or talk about the accident need to be considered when assessing the financial impact of an accident. Repair or replacement of damaged material or equipment. This includes the time to order, deliver, and test the new machine following the accident.

Training replacement workers. Recruiting and training temporary or permanent workers-and all the costs incurred by administrative personnel-need to be considered. Overtime. Extra costs of employee overtime to make up lost production frequently occur after a workplace accident.

Other indirect costs of an accident also result in loss of productivity, including the following: Foreman's diverted activity. Supervisory wages for time attending to the accident must be assigned to the total costs of the accident. Wages spent on reduced production. The cost of wages for an injured worker's return to a job could also be a factor, if the worker's performance decreases. Clerical supervision and accident investigation. Filing accident and investigation reports to insurance company and regulatory agencies, and the expense of accident investigation and recommendations for preventive measures, are to be considered. Remedial and compliance costs for equipment safeguards. Following the event, response to regulatory hearings and equipment modifications for compliance can be costly, including special safety training, procedures, and monitoring of results as directly related to the accident.

Cost of criminal negligence

There is a potential cost experienced by high-level executives who might be found guilty of violating workplace safety regulations. In the United States legislation allows senior executives to be goaled for breaches of workplace health and safety and THEY HAVE BEEN! Consider the outcome of the tragic 1991 fire at Imperial Food Products' poultry processing plant in North Carolina. In what has been one of the worst industrial accidents in the state's history, 25 workers were killed and 50 injured because of illegally locked or blocked doors. A federal judge ordered the insurance companies to pay US$18.1 million to settle claims and, as a result of the safety violations, Imperial Food's owner was sentenced to 19 years in prison on 25 counts of manslaughter.

In yet another example, from the litigious United States, the Illinois Supreme Court ordered five senior officials of Chicago Magnet Wire Co. to stand trial on criminal charges that they were responsible for job-related injuries to their employees. The decision stems from charges against the officials of allowing more than 40 workers to become ill from exposure to a dozen hazardous chemicals. Legal experts say the decision is likely to lead to additional charges against corporate officials in worker injury cases in other states.

Before we get to smug in the Antipodes it is important to realise that the State of Victoria has “Industrial Manslaughter” as a charge and there have been managers charged. In most other states the jurisdiction does allow for imprisonment; although we are yet to see it applied. Maybe it is “about time”.

The real story

According to the National Safety Council, the indirect costs of industrial accidents are approximately four times the actual direct costs. In other words, the real costs of workplace injuries are several times the calculable workers' compensation and employee disability payments. When this calculation is combined with the potential costs incurred by executives in defending against criminal negligence, the true cost of workplace injuries becomes staggering. These costs, however, are unnecessary. A recently released study notes that about 85 percent of all workplace injuries and fatalities can be avoided. The findings state that most workplace accidents and injuries stem from human error or inattention that can be prevented by making sure employees have been properly trained for their jobs, by introducing and requiring all employees to adhere to safe workplace practices, and by getting managers as well as the operational staff to make a genuine commitment to worker safety Interestingly, the factors listed are among the components the NSW Consolidated Regulation of 2001 advocates.. Study after study has shown the crucial role that management plays in effective safety programs. Such concern is manifest in a number of ways: appointment of a high-level safety officer, rewards to supervisors on the basis of the safety records of their subordinates, and comparison of safety results against preset objectives. Management's good example completes a loss control program. If hard hats are required at a particular operation, then executives should wear hard hats too. If employees see executives disregarding safety rules or treating hazardous situations lightly by not conforming to regulations, then they will feel that they also have the right to violate the rules. In short, organisations show their concern for loss control by establishing a clear safety policy and by assuming responsibility for its implementation.

The biggest “stumbling block” is the manager who makes “all the right noises” but in their observed practice continues to place other factors, such as productivity, as a higher priority Operational staff see this dichotomy (some might go so far as to suggest hypocrisy) and behave accordingly in their own approach to safety management.

Essential steps to safety

Investing in a safer workplace cuts the expense of treating injured workers and helps companies control insurance premiums and prevent workplace accidents. Equally important, effective safety standards in the workplace boost employee morale by conveying the message that the company cares enough about its people to protect their health and safety. Safety problems can be easily and inexpensively avoided by taking the following steps:

Tighten up housekeeping.

It sounds simple, but such activities as keeping floors swept and obstacles out of the way make a major contribution to reducing the number of accidents in both factories and offices. Pay special attention to seemingly harmless items that can easily become hazards e.g., water spilled around a water cooler or pathways obstructed by wires or boxes. Conduct regular safety checks. Unless your company is large enough to hire a full- or part-time risk manager, consider retaining a safety consultant to inspect the premises once or twice a year. To find one, consult your insurance company. It may offer to do the inspections or suggest other specialists. The cost is typically $300 to $400 for one day--the time required to make a safety inspection at most small companies. Conduct regular fire drills. If you retain a safety consultant, ask for suggestions on the frequency of fire drills. As a rule, businesses need drills two or three times a year, but some types of companies handling flammable materials should hold drills more often.

Train employees to be safety conscious.

Alert workers to areas that pose potential safety risks, such as stairways and light fixtures. Many accidents can be avoided by orienting new employees to their environment or by reminding current workers of past accidents and procedures for avoiding them in the future.

Continuously improve safety training. Few companies would let untrained employees operate heavy machinery, but a surprising number allow them to use everyday office equipment that can also cause injury. Training basics include instructing all employees in safe machine operation, stating safety procedures in employee handbooks, and supporting employees in first aid training offered by the St John’s Ambulance or Red Cross etc. Each of these suggestions can be implemented inexpensively and has reduced workplace injuries in organisations of all sizes.

Real-life benefits

An example of this positive outcome was seen within a local government organisation that I provided services to. The organisation cut workers' compensation premiums by more than $200,000 after experiencing a significant reduction in accident frequency and severity. In addition far more structured programs for assistance with workplace reintegration were implemented which resulted in a Return to Work Rate of 100% over three years. Unfortunately this organisation made a decision to modify its approach to safety management and is now confronting increasing costs again. As your company undertakes a safety program, consider the cost/benefit analysis outlined above. Beyond mere regulatory compliance, you have a number of options regarding the degree to which your company invests in employee safety. Underlying these efforts is a conviction on the part of many firms that it is morally right to improve job safety and health; and that doing so will enhance the productivity and quality of work life of employees at all levels.

EPF contributions will be reduced

The govt has announced that w.e.f. Jan 09, EPF contributions will be reduced from 11% of their salary to 8% for 2 years. This is to give employees more disposable income (YES! The word is “disposable”) to increase household spending to stimulate the economy. It will be applied to all employees. Those who wish to maintain the 11% contribution have to apply thro a special form.

Several bodies including CUEPACS are against this reduction as it means less household savings.

If the reduction goes through in Jan 09, we have a great opportunity to get our friends/prospects/investors to put the 3% into a monthly DDI so that they will not spend it.

If they do not want us to know how much they earn, let’s be sensitive & suggest a figure.

As our minimum monthly investment is RM100, it would mean that those with monthly salary of RM3,333 will have an extra RM100 net take-home pay. Those who earn RM3,333, do encourage them to invest more, suggest amounts like RM300 or RM500 p.m.

& use amounts like RM338 or $555, reason we give them is that it will be easier to identify the amount in our bank account. Every additional RM38 or RM55 u can get will increase your total monthly DDI be a lot!

Advantages of DDI Sales:

Ø Building up your DDI sales will build up a passive income for u gradually as the monthly amounts are automatically invested with no further effort from u or the investor.

Ø Investors will not feel the pinch as the monthly amounts are small but the cumulative amounts eventually will be substantial.

Ø U can ask for increase of the monthly amount when investors have more income later.

Ø U can also ask for additional DDI investments when investor has a new child, etc.

Ø Investors get to accumulate more units in the long run becoz of DCA (dollar-cost-averaging).

Mental Health


What is mental health
Mental Disorders are health conditions that are characterized by alterations in thinking, mood and behaviour associated with distress and or impaired functioning.
Mental Health is viewed as bodily systems working in successful functioning, harmony and balance.

Imbalance disharmony of the bodily systems may cause illness which originated from physical, psychological, nutritional intake, workplace environment or spiritual influences within the workplace.
If at workplace, an employee is subjected to constant or continuous workload or exposed to stressful environment, the body’s ability to fight infection is reduced


Stress all of its forms, from mental to job-related, affects physical health in a variety of ways including social health and lives

Daily social interaction in the workplace between superiors and subordinates or neighbourhood indicates the level of good mental harmony.

Most people will experience stress at work. However when an employee or person or group becomes chronically affected by stress, there will usually be characteristic signs and symptoms that can provide clues.

Every employer should and or need to develop policies and procedures to ensure that their approach is systematic and thorough. A specific policy relating to mental health at work is advisable by means of preventing problems through eliminating and minimising stress at source.

Adequate and suitable education or training programme and proper communication channel to assist individuals or groups who are experiencing the effects of stress to channel and seek redress. Setting up of counselling unit (either external counsellor) is a forward step in channelling any grievances.
Early intervention and progressive support from the management and promotion of positive health can minimise stress at workplace.

Other likely step should be taken by the Employer to minimise stress at workplace:
a) avoid or minimise autocratic management styles
b) due respect to ideas by workers
c) treat the workers well not like robot. They have feelings.
d) Create conducive working environment – flexible management style, management by walking etc
e) Value and tap the human capital capabilities at the organisation
f) Provide micro credit facilities if possible to ease any financial burden
g) Training and re-training to facilitate higher productivity
h) Promotion by merit and avoid office politics
i) Reward to performing staff
j) Create or provide more welfare programmes

Other precautionary steps in keeping your mental health in good shape
Always think and speak positively. You will get what you think. Your “think” will develop into thought that will eventually drive you either positive or negative.
Other proactive steps that every individual can do to avoid minimise and eliminate stress
Depression eclipses other chronic disease for poor health status.
Food additives may cause hyperactivity thus leading to heart complication.
Portion-control dishes or food intake helped people from obesity.
Waist size of 40inches for men and 35inches for women increases the risk for heart related diseases and diabetes.
Unfairness will increase heart risk and cannabis or tobacco disrupts brain centre.
Sleeping pills can cause strange behaviour like driving and eating while asleep or hallucinations.
Always practice good behaviour at workplace and neighbourhood.

Occupational Disease

Work related diseases, unlike occupational disease which has a direct cause-effect relationship between hazard and disease (eg. asbestos-asbestosis), are defined as multi factorial diseases among a working population when the work related diseases environment and the performance of work contribute significantly but as one of a number of factors to the causation of disease. Work related diseases which are partly caused by work, and/or aggravated, accelerated or exacerbated by occupational exposures, and/or the cause of impaired work capacity can also be partially caused by adverse work conditions. Personal characteristics, genetics, lifestyle and environmental and socio cultural factors usually play a role as risk factors for these diseases.
In 2004, Ass. Prof. Malcolm Sim of Monash University highlighted that in contrast to work related injuries that is traumatic, work related diseases is something which will tend to come on some time later after a latent period, like a cancer or respiratory disease. Hence, it is difficult to link the disease to the work environment because of the time lag although based on studies in Australia, work related diseases are very much common. For example it has been suggested that between 15 to 20% of adult asthma could be occupational in origin. He was also of the opinion that respiratory tract cancers, bladder cancers and leukemia can be related to work practices but they are not well recognized as work related diseases due to the disease itself having multiple causes.2

Common work related diseases are musculoskeletal disorders including Hand Arm Vibration Syndrome and Repetitive Strain Injury, heart disease, high blood pressure, work related cancer, respiratory/lungs diseases, skin disease, psychological disorders, stress diseases, and deafness. A specific example of work related diseases and the percentage in a population can be described by a survey conducted in the United Kingdom (UK). The Occupational Physicians' Reporting Activity (OPRA) surveillance scheme for occupational physicians which was conducted for 4 years (1996-1999), estimated 43,764 new cases of work-related disease have been reported. Musculoskeletal conditions make up nearly half (49%) of all cases; mental ill-health and skin disease account for 20% each, with respiratory conditions (8%) and hearing loss (5%) seen in lower proportions. Overall, eight of 42 diagnoses made up four-fifths of the new cases reported by occupational physicians. These were hand and arm disorders (8052 estimated cases), contact dermatitis (7104), disorders of the lumbar spine (6000), anxiety and depression (4788), work-related stress (3336), hearing loss (2100), elbow disorders (2040), and asthma (1680). Dermatitis and hearing loss were most frequent in manufacturing industries, lower back complaints in healthcare, and upper limb disorders in automotive manufacture. Psychiatric illnesses presented a different pattern, mainly affecting those in health, education and social service.3
“Decent Work Must Also Be Safe Work”

INCIDENT INVESTIGATION


An incident is inevitable if there are no controls in place. An incident will not always involve personal injury. An incident may also be property damage, environmental damage or a near miss.
Near Miss - an incident which could have caused serious bodily injury or extensive damage to property, but in the particular case did not. These are recorded so that steps can be taken to prevent a recurrence where injury or damage may occur.

It is important to investigate all incidents because
Investigation allows for the identification of incident causes to establish preventative strategies
It provides a point of reference to establish trends in incident causes.
It encourages organisations to communicate incident information and learn from past incidents
It also shows that the organisation is taking appropriate actions to prevent recurrence of the incident
Incidents may become common law cases up to three years later. If not properly investigated at the time of the incident, details fade and memories become confused.


Who should investigate the incidents?
This will be dependent on the seriousness of the incident and the potential or actual loss, damage or injury involved. The investigation team may include
Immediate supervisor
Workplace Health and Safety Representative/Officer
Person/s involved
Head of School/Element
Health and Safety Specialist and
External consultant (if applicable to the incident)


The Strategy of an investigation includes four stages
Gather information and establish facts
Isolate the essential contributory factors
Determine corrective actions
Carry out corrective actions


You need to ask 6 questions:
1. Who was involved?
This includes people both directly involved (injured or eye witnesses) and those that may be able to contribute useful information about facts surrounding the incident; eg supervisors who know what “usually” happens, trainers of persons involved, peers etc.

2. What happened?
This includes the equipment involved, the processes undertaken, systems, and what happened before and immediately after the incident. What “action” led to this incident? eg fall, slip, long-term exposure, hitting. What was happening, what task was involved?

3. When did the incident occur?
When was an incident noticed? When did the person involved start work that day? How much experience had they had in the task? It is important to identify here the date, time of day and other psychosocial issues that may have contributed to the event.
Psychosocial issues that may impact on when and incident occurs includes
- Has the person involved just worked extended hours?
- Did the incident occur at night/ on a night shift?

4. Where did the incident occur?
Have there been previous incidents in this particular area? What was the investigation outcome for previous incidents? Are there any physical aspects of the environment that may have contributed to the incident?
Look for both big and small differences and aspects of the environment.

5. How did the incident occur?
What was unusual about the event? How could the incident have been prevented? This may lead to the direct cause of the incident.

6. Why did the incident occur?
Was the person involved properly trained for the process undertaken? Were there safety procedures in place? Has a risk assessment been conducted for the task in question? Why were safety systems, procedures, training not followed? or Were systems followed but they failed?
The “atmosphere” at the time of the incident and psychosocial issues will impact on why an incident has occurred eg is it the last day before a holiday, is there some reason why the importance of safety precautions may be diminished?
Taking photos of the incident site and notes immediately after the incident occurs helps to formulate a good investigation.

HAZARDS AT CONSTRUCTION SITES

Type Of Hazards That Required Attention



A. Scaffold

Scaffold was identified as one of the main hazards in construction sites. Checklist of
hazards regarding scaffold works at site include items such as competence person,
access, condition of elements, ties and working platform, guard rail, toe boards,
signage and inspection. Among the items that require attention are: -

1. Additional precaution condition i.e. intermediate guard rails are still lacking and
only received 156 scores which are the lowest among all other hazard in
scaffolds.
2. Barrier or warning notices are most of time unavailable or insufficient to warn
people using the incomplete scaffold received 170 scores.
3. Insufficient or never conducted the periodical inspection of the scaffold. The
score obtained for this hazard is 190.

Howev er, item for condition of scaffold that secured to the building or structure
to prevent collapse for most of the sites visited was quite satisfactory with the score
of 254 (highest score).



B. Power Access Equipment

Hazard in association with power access equipment identified include competence
person, skill of operator, equipment security, guard rail, barrier and power supply.
Item such as precaution taken to prevent people being struck is being identified to be
having the lowest score of 70. However, overall ranking for power access
equipment still satisfactory with rank 3 that demarcate additional works still require
to satisfy o riginal specification.



C. Ladder

Hazard in association with ladder include item such as ladder condition, position and
foundation, suitability and prevention to slipp ing sideways. The cond ition of all
ladders receive the lowest score with 336. The other hazard identified was the
preventio n of ladder to slip sideways or outward with 380 points. However, overall ranking for ladder still satisfactory with rank 3.


D. Roof Works

When dealing with roof works, precaution to exclude people from area below roof
works and other additional precautions to stop debris falling onto them were the
lowest score among all with 136. It is followed by barriers and guard rail or cover to
provide people pass or work near fragile material or etc with second lowest score
156. Overall ranking of roof works are 2 due to ex tensive works required to satisfy
the safety regulation for the said works at sites.



E. Manual Handling

Hazard in association with manual handling was identified as risk of injuries due to
manual handling. It is being identified to be having the lo west score with 336.
Howev er, overall ranking for manual handling still satisfactory with rank 3 that
demarcate additional works still require to satisfy original specification.


F. Plant And Machinery

Most of the hazards in association with plant and machinery were identified to be
more significant at site involve in infrastructure works and industry building.
Howev er, overall rank for these groups of hazards are quite satisfacto ry with overall
ranking of 3 which demarcate on ly additional works required to satisfy the
specification. Hazards that need to take note by all community at sites in association
to plant and machinery shall be condition of guards for any dangerous p art and the
precaution on the dangerous part.



G. Excavation

Excavation was identified to be the major hazards in construction sites involve with
infrastructure works. For other sites, the excavation had already completed at the
time of survey. Among the items survey are method and adequacy of support for
excavation, slope, access, guard, barrier, signage and spoil. From the analysis, the
following hazards in association with excavation are identified to be the most
significant for infrastructure works at sites in chronological order: -

1. Unavailability of insufficient stop block or signage to preven t or warn tipping
vehicle falling in with score of 30.
2. Inadequate guard rail to prevent people falling in with score of 35.
3. Edge of excavation stored with material, plant, etc with 45 scores.




H. Fire And Emergency

Fire and emergency was detected as one of the most common hazards in all
construction sites. In the earlier part, we had defined this hazard to be one of the
common hazards that had great exposure to the workers at construction sites.
Among the items survey are storage for flammable substances, suitability and
availab ility for extinguisher, ignition sources, exit, alarm, awareness and emergency
procedure. From the analysis, the following hazards associated with fire and
emergency at sites are identified in chronological order from lo west score to the
highest: -


1. Emergency procedure of evacuating during fire alarm was detected to be the
lowest score with 224 scores and rank 1 which demarcate no way to satisfy the
original specification. During the sites visit, it was realized that majority of the
project sites do not have any evacuating procedure during fire alarm.
2. Regarding the awareness of emergency procedures by workers at sites, it had
scored 252 scores which are the second lowest in the checklist o f fire and
emergency and rank 1 as well. Most of the sites had a total ignorance to the
importance of procedure during fire alarm not even by the workers at sites but
the upper management as well.



I. Hazardous Substances

Hazard related to hazardous substances was identified having the lowest score with
420 as protective equipment used in handling the substances. However, overall
ranking for hazardous substances still satisfactory with rank 3.

J. Noise

Hazard associated with the noise was identified to be the appropriateness of hearing
protection used by worker which receive the lowest score with 168 and rank 2 which
demarcate that extensive works require to achieve or meet the specification.

K. Protective Clothing

Hazard regarding the protective clothing was identified to be the attitude of workers
toward using this equipment which score the lowest with 308 and rank 2. Other
items surveyed were the supply and condition of the personnel protective equ ipment.

L. Protecting The Public

For protection to the public, signage to warn public was detected to be the lowest
scores which had obtained 350 scores and rank 3. Overall ranking for this hazard
was still satisfactory with rank 3.


Within the scope of the study the following conclusion can be made:-

i. Work Place Environment

Within the area of th e scope of project regarding site environment (in chronological
order) the following items would require a great attention for our safety practitioners
due to the lowest scores obtained ; -


1. Condition of holes and opening to prevent fall
2. Site tidiness
3. Arrangemen t for debris collection

ii. Common Hazards At Construction Sites

It can be concluded that in general for most of the project sites around the study area
had a common hazards as follows (in chronological order from highest frequency to
lowest);

1. Protective clothing
2. Noise
3. Fire and emergency

It was observed that most of the project site workers within the scope of study
had a low level of awareness toward using the personal protective equipment. Even
for the employer, the supply for this equipment is seen to be quite inad equate
compare to the quantity of workers at sites. This situation still required an enormous
improvement to catch up to the acceptable level of safety and health practice at site.

PENGURUSAN INSURANS DALAM SEKTOR PEMBINAAN



Penyelidikan ini bermatlamat untuk mendapatkan kefahaman tentang
penggunaan insurans dalam industri pembinan di Malaysia. Liputan insurans yang
mencukupi adalah penting untuk melindungi kewangan dan sumber bagi sesebuah
organisasi. Syarikat-syarikat insurans membangunkan sumbernya melalui premium
insurans. Kepentingan pengurusan insurans dalam industri pembinaan adalah sebagai
langkah keselamatan dan supaya dapat meningkatkan kemungkinan bagi menebus
kerugian yang dialami melalui insiden yang dipersetujui di dalam polisi insurans.
Objektif penyelidikan ini adalah untuk mengenal pasti faktor-faktor yang
mempengaruhi harga premium, mengkaji jenis-jenis tuntutan yang timbul daripada
aktiviti pembinaan dan menyelidik prosedur membuat tuntutan insurans. Bagi
menjawab objektif-objektif tersebut, borang soal selidik telah diedarkan kepada pihak
kontraktor dan syarikat insurans, dan temu ramah semi berstruktur telag diadakan
dengan pakar dan wakil daripada syarikat insurans. Kemuncaknya, penyelidikan ini
mencadangkan beberapa tindakan yang boleh diaplikasikan oleh pihak kontraktor
untuk mengurangkan insiden di tapak bina.



Secara keseluruhannya dapat dirumuskan bahawa dalam praktis pembinaan
